As your recovery journey unfolds, we maintain contact with all relevant parties and track every detail to ensure you’re receiving the support you deserve every step of the way.

CASE MANAGEMENT:
HOW IT WORKS

PHASE 1: ASSESSMENT

To begin the case management process, one of our master’s level therapists will meet with you and your loved ones (if appropriate) to discuss your recovery journey so far.

During this informal assessment, we’ll gather information about what led you to treatment, what challenges you are facing now, your long term goals, and what modalities that have proven effective/ineffective for you in the past.

PHASE 2: CREATING A RECOVERY PLAN

Once we are familiar with your immediate and long term needs, we help you create a comprehensive recovery plan.

This collaborative process involves identifying all types of support you currently need and then narrowing down the best options for your unique situation. At the completion of this phase you’ll have a clear understanding of your initial recovery support system and a schedule for all appointments, meetings, etc.

PHASE 3: ONGOING SUPPORT

As you progress through your recovery journey, we’ll continue to meet with you regularly and track all relevant details to ensure everything is going as planned.

When new problems or goals crop up, we’ll work with you to reevaluate your recovery plan and make changes when appropriate. If any new services are needed such as specialty therapists, dentists, or nutritionists, we’ll help you find the best available resources for your evolving needs.
